Tiny Homes
Sulzbacher’s newest short term housing program, a partnership between Synergy Steel, Sulzbacher, JFRD PATH, and the Jessie Ball duPont fund, is located at the 611 East Adams Street campus. This program offers an innovative approach to serving the individuals with barriers that make it challenging or impossible for them to reside in traditional congregate shelter programs. Historically, there have been few resources for those who are unable to accept or stay in traditional congregate shelter programs. This program consisting of 10 Tiny Homes adds capacity to take 10 of our most critically vulnerable clients off the street and into safe, dignified, comfortable temporary housing in a way that does not have the limits of traditional congregate shelter. Participants are assigned their own private tiny home unit and have access to on-site showers and services through the Urban Rest Stop, daily meals, intensive case management, and the full array of on-site services including Duval Family Health medical, dental, vision, behavioral health, and substance use services. All participants are selected and referred by the JFRD PATH Team. Program participants generally have extensive history of homelessness (5 years plus) and one or more of the following: physical disability, severe and persistent behavioral health diagnoses, substance use disorder, developmental disability
Chop
Sulzbacher’s CHOP program (Chronically Homeless Offenders Program) is a unique partnership between the Jacksonville Sheriff’s Office, the State Attorney’s Office and the Public Defender’s Office. CHOP offers people who have been homeless for a year or more, the opportunity to receive permanent housing placement with wrap-around supportive services to maintain that placement. By targeting the most frequent misdemeanor offenders in Jacksonville, Sulzbacher is able to break the cycle of chronic homelessness and nuisance crimes for these extremely vulnerable individuals.
For more information about the CHOP program, call (904) 394-4950.

By partnering with the Veteran’s Administration, Changing Homelessness, and COJ’s Homeless Veteran Reintegration Program an array of services are offered to serve Veterans and their families.

The Urban Rest Stop provides a safe respite from being on the streets, with off duty JSO, an indoor air conditioned tv room and outdoor covered deck to escape the elements, daily showers for men and women, lunch and dinner 365 days per year, mail services, laundry, coordinated entry services offered by Changing Homelessness, resume writing, job search, and interview prep offered by the onsite Goodwill job junction, medical, dental, vision, substance use, and behavioral health services offered by Duval Family Health, and more.
