Permanent Housing

Sulzbacher offers several options for people experiencing homelessness or people at-risk of becoming homeless to find permanent housing placement. Housing placements are offered at Enterprise Village and Sulzbacher Village, as well as several scattered-site locations throughout the city. Most housing programs require referrals from Duval County’s Coordinated Entry System, which is operated by Changing Homelessness. The first step to finding out what programs you may qualify for is to complete an assessment with Changing Homelessness staff at the Urban Rest Stop on Mondays beginning at 9:30am, on the Mobile Outreach Bus, or by contacting Changing Homelessness directly at (904) 354-1100 or outreach@changinghomelessness.org.

Step 2: DOCTORS WHO CARE

Village Healthcare


Sulzbacher has been providing healthcare for over 27 years. A brand new, state-of-the-art 30,000 sq. ft. healthcare facility will offer the space needed for the comprehensive services that will continue and expand in the new location.


• Primary care, optical care, dental services, and a pharmacy, delivered by doctors and nurses who care.

• Behavioral health counseling with our psychiatrists and counselors.

• Referrals to specialists.

• A staffed respite unit for men recovering from hospital discharge.

Training Center and Manufacturing Plant


Providing opportunities to gain real world training and skills, setting men up for success.


• A Training Center, in partnership with FSCJ, offering trainings and certifications in the jobs our community needs.

• A manufacturing plant offering full-time employment – focusing on teaching the skills needed to build the affordable housing our community desperately needs.

Sulzbacher Village

A Brighter Future for Women & Families

Sulzbacher Village is an award-winning, national model for ending homelessness for women and families, providing dignified services that include:


• permanent supportive housing

• emergency housing

• pediatric healthcare

• therapeutic early learning center

• job readiness and job placement services


All services are offered onsite in a 92,000 square foot community located in the Brentwood neighborhood of Jacksonville. Sulzbacher Village is located on a major bus line and within walking distance of the elementary school and grocery store.

Housing

Sulzbacher Village can accommodate approximately 310 residents. The housing opportunities include:


• 70 studio or two-bedroom deeply affordable, permanent housing units – eligible residents sign a yearly lease that can be renewed. Each unit is fully furnished.

• 46 emergency units for women & families in crisis

• 8 women veterans’ units


Healthcare

For the approximately 80 children living at Sulzbacher Village on any given day, healthcare is just an elevator ride away. Operated in partnership with Wolfson Children’s Hospital, the Pediatric Health Center also welcomes and provides services to children from the surrounding community.

Hope

Hope is essential for addressing the challenges facing homeless women and families. Sulzbacher Village offers solutions by providing wrap-around services to meet their needs, including:

• Children’s Services: Sulzbacher Village has children of all ages, birth – 17, and provides services focused on both educational success and healthy development.

 

A comprehensive Children’s Program for school-aged children includes tutoring, mentoring, summer camps and cultural enrichment activities and field trips.

 

The Crawford Early Learning Center is a high-quality early learning program that includes a therapeutic, developmentally responsive approach to help children overcome the trauma that many have experienced. Learn more about CELC.




• Veteran’s Services: Eight emergency housing units provide a safe place to get homeless women veterans off the streets. Special programs and accommodations specifically suited to help them address their unique challenges are provided to help them move on with their lives.

 

• Goodwill Job Junction: On-site Job Readiness and Job Placement services are offered through Goodwill Industries. Residents receive job skills and training for an assortment of positions, as well as help with resumes and interviewing skills.


• Case Management & Rental Assistance: At the core of each of Sulzbacher’s programs is Case Management and Supportive Housing. These services are individualized to meet the unique needs of the person and include advocacy, empowerment, guidance, encouragement and referral to outside resources.
