Housing and Hope on Wheels
Sulzbacher’s Housing and Hope on Wheels Bus delivers vital services directly to those experiencing homelessness in Duval County. By traveling to existing service locations across Jacksonville and The Beaches, the bus helps remove one of the greatest barriers to services that our homeless neighbors face: transportation.
Our Hope Team meets people where they are and connects them with the resources they need to take steps toward stability. Services include housing and benefits assessments, healthcare referrals, and access to basic necessities including hygiene kits and socks.
Collaboration is at the heart of this work. Through strong partnerships with other nonprofits, Housing and Hope on Wheels is able to expand its reach, connect individuals to a broader network of services, and create a pathway to stability and self-sufficiency. This mobile outreach program is a vital tool in our mission to end homelessness, one connection at a time.
Healthmobile
Sulzbacher’s newest addition to its health services is a mobile unit providing primary and behavioral healthcare to persons experiencing homelessness in Jacksonville’s downtown core. The Healthmobile is an expansion of the Urban Rest Stop at Sulzbacher’s Downtown Campus. The Rest Stop is a partnership between the City of Jacksonville, Sulzbacher, and Mental Health Resource Center, and is a place for people experiencing homelessness to go during the day to receive services including job and housing placement.
The Healthmobile is comprised of a waiting room and two private exam rooms to provide dignified care to people who are the most vulnerable and are disconnected from existing services. This program removes the barriers of transportation by bringing medical care and case management to those in urgent need of the services, restoring health, hope, and self-sufficiency. In addition to medical services, following assessment and intake, clients will be paired with appropriate services to obtain housing referrals for case management and permanent housing.
